By clicking on the "I agree" button below I understand that I am signing this waiver electronically and that my son/daughter has my permission to participate in the Saint Louis Youth Football Organization (SLYFO), and that I am unaware of any physical and/or mental deficiencies that would in any way endanger him/her by participating in this program. Furthermore, I release and hold harmless from any liability for any physical or mental injury, aggrevation of any pre-existing condition, or any other harm or loss of any nature which may be sustained as a result of my son/daughter participating in the SLYFO, the SLYFO, coaches, volunteers, referees, organizers, and anyone else associated with the SLYFO, and any Public School location in which he/she participates, except that caused by willful, wanton, or reckless conduct. Furthermore, I give permission to the SLYFO, their coaches, and staff, to secure emergency medical and/or emergency surgical treatment for the above named minor child while participating in the SLYFO Youth Football Program. Non-emergency medical treatment or elective surgery is not included in this authorization.

By clicking the "I agree" button below I/we give the Saint Louis Youth Football Organization (SLYFO) permission to pull my child's grades for eligibility purposes, and hereby release Saint Louis Public Schools from any liability regarding the release of those grades to the SLYFO. Furthermore I understand that the SLYFO's guidelines for eligibility are in line with Saint Louis Public Schools and that if my child receives an F in any class for that given week they will be ineligible to play during the game that week, but are still required to practice as normal and show up on the sidelines wearing street clothes and their jersey to support their team.

Refund Policy: Refunds for youth who decide not to participate are only issued in the first 14 days of practices beginning unless extenuating circumstances exist at the SLYFO board’s discretion.
